Qualifications & Requirements - Active LPN license in New Mexico (or compact state eligibility) - Licensed Practical Nurse required
- Minimum 1 year of experience in long term care, skilled nursing, or rehabilitation (LTC experience preferred)
- Current BLS certification (AHA preferred)
- Strong communication, documentation, and interpersonal skills for resident-centered care
- Ability to work effectively as part of an interdisciplinary team (RN, therapists, social work)
Primary Responsibilities (LPN / LTC Nurse) - Provide direct nursing care to residents according to facility policies and LTC best practices
- Administer medications and treatments safely and document per protocol (med pass, PRN documentation)
- Monitor and document residents' health status, notify nursing leadership of changes, and participate in care planning
- Assist with activities of daily living (ADLs) and support rehabilitation exercises alongside therapy staff
- Collaborate with RNs, physicians, and interdisciplinary team members to ensure high-quality resident care
- Maintain accurate, timely electronic medical record (EMR) and paper documentation
